DOWNLOAD AN AO REGISTRATION FORM

Athletes:

An athlete can become a member for the entire year (which is the most cost effective), or for part of the year.  Please select the membership period which best suits your purposes. Athletes who register for part of a year may upgrade to full membership at any point.

Each of the athlete registration forms comes in two parts.
The first part is the Athlete Registration form itself, and everyone who registers with AO needs to fill one in. This form must be co-signed by the Registrar of your Track Club (unless you are registering as an Independent Member or Unattached Official). Your form must be accompanied by the appropriate fee before it will be processed.

The second part of the form is a Power of Attorney, and should be completed only by those athletes wishing to appoint a Power of Attorney (if you aren't sure about a Power of Attorney ask your club Registrar or Coach). Essentially, the Power of Attorney form is a complicated but important document which allows someone in your club (or a designate) to sign waivers on your behalf. This is a convenience which will relieve many administrators of the task of getting entry form waivers signed everytime an athlete enters a meet.

Coaches, Administrators, Staff and Board Members

Coaches, Administrators, Staff and Board Members who have previously submitted a Police Record Check are NOT required to submit a new one for 2009. Instead they will have to submit an Offence Declaration form which can be downloaded from this site (below).

New Coaches, Administrators, AO Staff and Board Members will have to submit a Police Record Check with their applications for membership. New membership applications that are not accompanied by an original Police Record Check will not be processed. Coaches and Administrators wishing to register in subsequent years will only be required to submit an Offence Declaration with their membership applications.

The Offence Declaration Forms and Police Record Checks are part of a sport wide initiative to screen volunteers who come into contact with our young athletes. If you have questions or concerns about the Volunteer Screening program, please see the Volunteer Screening document, which now forms part of the AO Policies and Procedures Manual.

Officials:

Officials forms are two pages long so they can list their levels of Certification. Officials are not required to submit a Police Record Check, but are required to fill in an Offence Declaration form with each yearly application for membership in AO. Forms can be downloaded below.

Volunteer Screening:

Athletics Ontario is always trying to make our sport safer for all participants. Since 2005, all volunteers (Coaches, administrators and officials) wishing to register a members of AO must undergo some form of Volunteer Screening. Some of this screening will happen at the club level, and some will happen at the AO level.
